A Case of Dermatolysis or Pachydermatocele

Present condition.?The tumour spriF.gs from the level of the left mamma and hangs down as far as the left great trochanter of the femur, its base or pedicle measures five inches in width and nine inches in girth or circumference. The length of the tumour from base or pedicle to its most dependent point is twelve inches, and the circumference of the tumour at its thickest portion measures fourteen inches. The tumour consists of a lax outgrowth of connective tissue, and feels like a long loose bag of integument, which can be separated on pulling it out from opposite directions, and the looseness of the contained areolar tissue allows the inner walls of the tumour to be rolled one upon the other. The tumour on being grasped between the fingers gives arterial pulsations, showing that it is well supplied with blood. The surface of the tumour presents the main characters of normal integument somewhat thickened, and the skin follicles are very distinctly marked. Cuticular sensibility is slightly diminished over the tumour. The tumour has never shown any tendency to ulcerate at any point, although it has often been torn by thorns and brambles when the patient was working in the fields. Operation.?A few days subsequent to his admission into hospital the patient was attacked with diarrhoea and dysentery which weakened him very much. He was also found to be suffering from internal piles, which were removed by clamp and cautery. He became anaemic, ;ind all idea of removing the tumour was abandoned for the time. After remaining many months in hospital he regained his health, and on the 23rd January, 1884, he was considered to be in a fit condition to
